New Leaf Program Program Details:   1. Participants work with a CHES certified lifestyle coach in a group setting to receive a 1-year lifestyle  change program that includes 16 core sessions (usually 1 per week) and 6 post-core sessions (1 per month). 2. Participants will be asked to document all food consumption in weekly food logs, as well as weekly exercise. 3. The New Leaf Program is based off a research study by the CDC, which showed that making modest behavior changes helped participants lose 5% to 7% of their body weight—that is 10 to 14 pounds for a 200-pound person. 4. These lifestyle changes reduced the risk of developing diseases such as type 2 diabetes by 58% in people with pre-diabetes.  Patients Will Receive:   1. Free Zumba classes   2. Gift card upon completion of program   3. Free snacks during class   4. Weekly raffle prizes  Patient Eligibility: 1. Participants must be 18 years of age or older. 2. Participants must have a BMI over 24. 3. 50% of participants must have had a recent (within the past year), documented, blood-based diagnostic test indicating they have pre-diabetes, or a history of gestational diabetes mellitus (GDM), according to one of the following specifications: Fasting plasma glucose of 100 to 125 mg/dl Plasma glucose measured 2 hours after a 75 gm glucose load of 140 to 199 mg/dl Alc of 5.7 to 6.4 Clinically diagnosed GDM during a previous pregnancy 4. Participants may be considered eligible without a blood-based test or history of GDM only if they screen positive for pre-diabetes based on the CDC Pre-diabetes Screening Test.
